Cancellation Policy
Registered participants may substitute another participant at no charge up to the day of the conference. You will need to send an email to Lindsey Hess with the new participant name as soon as possible to coordinate meals and participant materials. Participants may cancel at no charge up to 7 days before the event begins. No refunds are available for cancellations 6 days or fewer before the event begins.

Shared Value and Success: Growing Resilient Communities
Purpose
The overall purpose of this Learning Session is to bring together Wisconsin Health Center teams to share and learn best practices that drive quality and create value.
Conference Objectives
At the end of the educational session, participants should be able to:
- Identify strategies that will encourage interdepartmental integration in their organization
- Utilize quality improvement strategies and targeted interventions to enhance the health and wellbeing of patients
- Describe ways to grow resiliency and enrich communication within your organization
- Participate in the learning community by networking with statewide peers
Accreditation
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of Interstate Postgraduate Medical Association (IPMA), and Wisconsin Primary Health Care Association (WPHCA). The IPMA is accredited by the ACCME to provide continuing medical education for physicians.
Credit Designation
The Interstate Postgraduate Medical Association designates this live activity for a maximum of 9.25 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
Performance Excellence Award
WPHCA is soliciting nominations for the third annual Performance Excellence Award, which will be conferred annually at WPHCA’s Fall Learning Session. The Performance Excellence Award will recognize and celebrate CHC performance improvement teams and the efforts those teams have made over the past year to improve the health of patients, improve patient experience with their care and/or improve the cost effectiveness of care.

Show-and-Tell Session
WPHCA will be hosting a Show-and-Tell Session at the 2017 Fall Learning Session (FLS) where Health Centers are encouraged to participate and share any improvements, current projects, PDSA cycles, or other team-based initiatives made in the last year or two related to quality. One breakout room will be open with tables and slide boards displaying Health Center posters where FLS attendants will have the opportunity to walk around, look at other Health Center posters, discuss methods and outcomes shared, and enjoy an informal networking environment.
