Learning Collaborative Purpose: Provide learning space, resources, and support for health centers in improving quality of care and outcomes for patients with diabetes.
Collective Action Goal: By the end of 2025 every participating health center will have established a diabetes action plan and begun implementing at least 3 key improvement efforts.
Collective Outcome Goal: By June 30, 2026, all participating health centers achieve a collective improvement of 5 percentage points (absolute change) on the UDS Diabetes Control Measure compared to June 2025. This will be accomplished by each health center achieving a relative improvement of 18.5%* in their individual health center's Diabetes Control rate. *This estimate is based on current data; exact figures will be calculated using baseline data at the start of the collaborative.
Format: The Collaborative will include a combination of monthly large group learning sessions , coaching calls, and accountability check ins on action plans. Evaluation will cover both the value and satisfaction of the T/TA delivery as well as diabetes outcomes.
