2022 WPHCA Teamwork Series: Exploring the New Frontier of Evolving Clinical Models April 20-21, 2022 Wilderness Resort Glacier Canyon Conference Center 45 Hillman Road, Wisconsin Dells, WI 53965 Registration: $30 Overall Meeting Objectives: As a result of this meeting, participants will be able to: - Describe how integrated team-based care models are evolving to include and expand the role of behavioral health providers, clinical pharmacy, nurses, care coordinators and population health managers
- Discuss key components of care management models that provide value to patients and payers
- Recognize how different and new technologies can be used to enhance the clinical care of various patient populations at their Health Center
- Develop intraprofessional relationships amongst Health Center attendees, including networking for clinical, behavioral health, operational, and population health leaders from Health Centers across Wisconsin.
- Identify clear next steps to review impacts of technology and teams on Health Center clinical models

the new participant name as soon as possible to coordinate meals and
participant materials. Participants may cancel at no charge up to 7 days
before the event begins. No refunds are available for cancellations 6
days or fewer before the event begins.
